Inpatient Care

Patients who have a fever of 37.0 ℃ or higher, diarrhea, etc. before your hospital stay,
please call the outpatient department you usually visit to inform of your conditions.
  • 1 When you check in, please submit the following items to Counter 3 (Admission and Discharge) at the 1st floor reception.

    • Hospital admission application form (Please make sure to include the information of your guarantor and joint guarantor.)
    • Questionnaire on Inpatient’s History/Certificate of hospital discharge if you have been hospitalized within the past 3 months at another hospital.
    • Patient ID card
    • Inkan/Hanko (Personal stamp or seal) (if available)
    • Health Insurance Card/Individual Number Card (My Number Card)
    • Other medical certificates (Eligibility Certificate for hospital/medical fee cap)
    • Consent forms
    • Medicine record
    • Questionnaire concerning Allergies (for the children under 15 years old)
    • Personal Data Book for Diabetes, Pacemaker and Blood pressure, etc.
  • 2 Please complete the hospitalization procedures on the day you check in to the hospital. However, if you are hospitalized at night , on a weekend, holiday, or from Dec. 29th to Jan 3rd, please complete the procedures during on the next week day business hours (8:30 a.m. to 5:00 p.m.).
  • 3Please refrain from parking your car while you are hospitalized.

Please remove the following things before your hospital admission:

  • Makeup
  • Nail polish/Gel nails/Acrylic nails/Fake nails/Nail extensions
  • Eyelash extensions/False eyelashes

Please be sure to put your name on all your personal belongings.
Hospital rooms don’t have a lot of space for personal belongings, so please only bring necessities.

  • Currently prescribed medications, medicine record or instruction sheets for your medicines
  • Change of clothes, underwear, sleepwear/pajamas, socks, etc
    Please also bring enough clothes to wear on laundry days.
  • Towels (2 bath towels, 3-4 hand towels) and Bath mats
    Rental and Laundry services are available.
  • Comfortable shoes with a non-slip sole for indoor use
    Footwear that expose the heels (eg. slippers, sandales, etc.) are discouraged.
  • Toiletries (soap/body soap, shampoo, hair conditioner, toothbrush, toothpaste, hairbrush, comb)
  • Plastic cup for oral care and teeth brushing
  • Stationary (pen/papers)
  • Tissues
  • Earphones/headphones for the TV
  • Laundry bag (plastic bag etc.)
  • Surgical mask

 

If you usually use any of the following, please bring them aswell:

  • Electric shaver
  • Dentures (and denture storage box)
  • Eye Glasses (and glasses case)
  • Hearing aid (and its case)
  • Walking stick
  • Cutlery (fork, teaspoon)(
  • Thickener for dysphagia management

Chopsticks, soup spoons are served with the meal.

  • Patients scheduled for an operation may need additional items depending on the operation you are having.
  • There is a Seven-Eleven convenience store in the hospital (open hours: 7 a.m. to 10 p.m.) where daily goods are sold hospital stay.
  • The most needed items for your hospital stay are also available at the vending machine on the 1st floor 24hrs/365.
  • Patients who prefer to use Rental and Laundry services or Diapers Service, please contact the hospital staff after being hospitalized.
  • Edged tools (scissors, fruit knives, razor, etc.)
  • A lot of money
  • Valuables
  • Jewelry (piercings, rings, etc.)
  • Alcohol (including non-alcoholic drinks)
  • Plants/Flowers
  • Animals/Pets
  • Cigarette/Cigarette lighter/Vape
  • Burnable substances
  • Electric kettles

The paid services provide patients and their families comfortable clean clothes, towels, diapers, etc.
Please inform the hospital staff if you prefer to use such services.

  • Patient Gown & Towels Rental and Laundry Service
    This includes a hospital gown (pajamas), a bath towel, a hand towel and a bath mat.
    Fees are charged on a daily basis regardless of the number of uses.
  • Diapers Services
    This service offers various types depending on patients’ condition.
    Please ask the hospital staff which of the services are suited for you after your hospitalization.

Your hospitalization costs include fees for:

  • Hospital stay (accommodation)
  • Medication
  • Cares (operation, treatment, examinations, diagnostic imaging, etc.)

Please note that the fees for meals, patients’ gown, diapers, private rooms, and medical certificates will be an additional fee.
